The Group's goal is to map out the integrated social and healthcare model for L'Alt Penedès and El Garraf

As a provider for the Catalan Health Service, Parc Sanitari will be involved in mapping out the social and healthcare model for L'Alt Penedès and El Garraf. It will do this as part of the Strategic Group in the Penedès Health Region's Health, Care and Wellbeing (SCiB) project. This project brings together the ministries of Health and Social Rights and Inclusion, L'Alt Penedès and El Garraf county councils, town councils in these two counties and various local social and healthcare stakeholders.

A project that's taking shape

The SCiB project kicked off in November with an initial working meeting, and one month later the Strategic Group which is to lead it has now been put in place. A new stage will begin in January 2026 to map out strategic strands and coordinate setting up working groups made up of professionals appointed by the participating organisations. These groups will identify initiatives, experiences and opportunities for improvement while also drawing up operational actions on the ground.

The primary objectives of this new stage include:

  • Assessing the needs of people in these two counties.
  • Mapping out the territorial model for integrated social and health care.
  • Drawing up a joint service portfolio tailored to actual local requirements.
  • Devising a single, shared work plan which puts people at the centre and promotes prevention, community care and continuity of care.

A project with a shared vision

The purpose of the Health, Care and Wellbeing project is to enhance the wellbeing of people by rolling out a model which addresses current service fragmentation and shifts towards coordinated, localised and people-centred care based on the two counties's demographic, social and territorial situation.

The initiative showcases the experiences, resources and innovation capacity of local stakeholders and seeks to step up coordination between services, care levels and institutions to unlock more accessible, preventive and efficient care.
